I agree to the General Release and Indemnification Agreement

Whereas Faulkner University is allowing the undersigned participant to participate in a sports camp sponsored
by Faulkner, in consideration of being allowed such participation, the undersigned person(s) agree(s) as follows:

I, being desirous of being allowed to participate in said sports camp and understanding that if I do not agree to the terms of this document that I will not be allowed to participate in the said sports camp,

and/or

I/we, as the parent(s)/guardian(s) of the person desiring to participate in the sports camp, and understanding that participation in the camp is expressly conditioned on agreement to the terms contained herein, on behalf of
myself/ourselves and the below named participant,

therefore, I/we for myself (ourselves), my/our heirs, successors and assigns, do hereby agree to release as well as indemnify, save and hold harmless the following persons/entities/organizations/corporations
(referenced hereinafter as Releasees/Indemnitees”)

Faulkner University, its Board of Trustees, its employees, officers, agents, servants and volunteers or others acting in or on the University’s behalf;

National Management Resources, its Board of Directors (or similar body if it has such), its owners, employees, officers, agents, servants and volunteers or others acting in or on its behalf;

Culinary Management Services, its Board of Directors (or similar body if it has such), if it has such, its owners, employees, officers, agents, servants and volunteers or others acting in or on its behalf;

from any and all claims of liability and/or damages or compensation that arise from or are in any way related to the participation in the above referenced sports camp by the named participant, including claims that are or might be predicated on or be related in any way to the negligent conduct of any of the Releasees/Indemnitees, and

do further agree to save, indemnify and hold harmless the said Releasees/Indemnitees from any and all claims of liability and/or compensation that arise from or might arise from or be in any way related to the actions or omissions of the herein named participant(s).
