Begin the CPT renewal process by signing below and clicking “submit”. Upon submission, you will receive one email from Faulkner University containing the renewal form.

Renewal Form
- You will receive the Renewal Form after you submit this request.
- The renewal form will need to be sent to the employer by the student using their preferred method.
- The form must then be completed and signed by the employer or the CPT renewal application will be denied. Typed script font computer signatures will not be accepted. The signature must be hand-signed by the employer or an electronic signature through a trusted, verified site such as DocuSign by the employer. Once completed, the renewal form must be sent to intladmissions@faulkner.edu.
