Legacy Camp Background Check Attestation Form

Faulkner is excited to welcome LEGACY members for the July 2026 camp event located on the Faulkner University campus.

Students walking As an independent organization, the LEGACY Board is solely responsible for establishing its own rules and guidelines, including the verification that all adult participants and volunteers who may have contact with minors during the camp event have successfully completed a criminal background check.

This form serves as official documentation that all volunteer staff affiliated with LEGACY for the upcoming July 2026 camp have complied with these background check requirements. Individual volunteers or representatives associated with LEGACY (including churches, groups, or other sponsoring entities) may also use this form to certify completion of such checks.

The Faulkner University Campus Safety/Police Department maintains responsibility for public safety within the jurisdiction of the campus for all faculty, staff, students, and visitors. Faulkner University and the Faulkner Police Department reserve the right to confirm that camp attendees have attested to the completion of required background checks through LEGACY. Submission of this form does not constitute a background check performed by Faulkner University, nor does it imply approval or endorsement of any volunteer by the University.

Adult attendees for the LEGACY camp who choose not to complete this form or comply with a background check approved by LEGACY may be denied access to Faulkner University’s campus and associated camp activities.
